SNOWDROP: SeNiors empOWred via big Data to joint-manage their medication-related Risk Of falling in Primary care

Among senior citizens (65+), falls are the leading cause of injuries. Medication is a major risk factor for falls. General Practitioners (GPs) struggle with fall-related medication management for older patients, as they lack tools to assess individualized risks. Furthermore, older patients themselves are not properly equipped to engage in the joint management of their medication. Within the SNOWDROP project we focus on the development and evaluation of a Clinical Decision Support System (CDSS) and a patient portal. Together these systems aim to enhance shared decision making between GP and older patient during a medication review aimed at reducing the patient’s fall risk. The CDSS uses a prediction model to identify older patients with a high fall risk and advises the CP during a fall-risk-related medication review. The patient portal can be used by the older patient in order to prepare for the consultation and to read information about their fall risk.

This project is a collaboration between the communication science department (UvA), the department of general practice (AMC) and the medical informatics department (AMC).
